Add Product and Insurance Type to payer_profile.sql

Update payer_profile.sql: added ProductTypeCD and InsuranceTypeCD fields. Max and min dates now group by these fields. This update was made after Lisa B. noticed that HPHC-PPO and TAHP-PPO members were not being pulled into Covered Lives SAM. The reason was that PPO enrollment data lagged behind other product types by 1 month. This View update should allow all members to flow in based on latest eligibility, regardless of Insurance or Product Type.
