Improve days covered calculation by using med equivalency

Hubbell, Harrison Taylor requested to merge hh_mdc_equiv into main

This mixes in Epic.Reference.MedicationEquivalency if available to help with unit conversion between dispensed units and dose units. Unit conversion issues have caused miscalculations in the past, and this appears to resolve some of them. There are still a number of known difficulties with calculating days covered. This makes it a little better.
