Questionnaire Assignments View

This view creates a single interface for identifying PROMs that were assigned to a patient, regardless of completion status. A questionnaire is a questionnaire, regardless of method of assignment, so this view allows users to see what got assigned without determining whether they need to use the "series" modality or the "appointment" modality.

This view will handle merging the two work streams into one and also de-duplicating any cases where a questionnaire was assigned using both modes. NOTE: This is only possible to determine for completed questionnaires.
