Use CoveredLivesPopulation as PatientMGBPatients

Use CoveredLivesPopulation as the source for PatientMGBPatients. The covered lives view is derived from the MGB patients logic, so this is functionally the same with a slightly slower refresh cadence. Acting as a proxy for covered lives will provide a massive performance boost for legacy queries that still reference MGB patients.
