Add Last Name, First Name to PatientDemographic view

Addition of separate First/Last name columns to the PatientDemographic view without spaces or special characters

Justification for request

On occasion a request may ask for a summary of demographic information on a list of patients but may contain very few identifiers- i.e. first name, last name, DOB. In cases such as these it might benefit us to have separate first and last name columns devoid of any spaces or special characters such as apostrophes, commas, hyphens, etc so that we can better utilize this view.
